New Student Information

Incoming students need to provide the following three health requirements to the Health & Wellness Center. Please click Student Health Documentation to complete and submit.

• Health History Form. Enter all pertinent information.
• St. Norbert College requires that you have two (2) measles, mumps and rubella (MMR) immunizations. Wisconsin residents can often find their immunization records in the Wisconsin Immunization Registry (WIR). You may waive out of required immunizations but will need to sign the waiver and agree to terms in the student health documentation form.
• Health insurance policy information.
• Any student under 18 years of age at the start of classes must have parent/guardian complete a consent for minor treatment of a minor form prior to receiving medical treatment at the SNC Health & Wellness Center. Contact the Health & Wellness Center to discuss and complete the necessary paperwork.
